Download the Profile Sheet here. Save it to your computer, complete the profile sheet and return via email to twinstalentagency@rogers.com.

 

Macintosh Users:

 

Download the Profile Sheet here. Save it to your computer and complete the profile sheet in Adobe. Alternatively, you can use the default "Preview" program. However, when you are finished completing the profile sheet in "Preview," you must choose "Print" from the "File Menu" and then use the dialog box's "PDF option" to save it in PDF format. See below. Saving it as a "PDF" allows windows users to be able to read the completed form.
 

 

 

 

 

 

 

 

Once you have completed the profile sheet and saved it as a PDF file, return it via email to twinstalentagency@rogers.com.
